We accept most major insurance plans and are in-network providers for the above insurance plans.

We also offer affordable self-pay rates for patients without insurance. If you have a high-deductible health insurance plan, you may want to consider our discounted self-pay prices below. When you book an appointment online, our system will perform an automatic check of your insurance and inform you of your co-pay or out-of-pocket responsibility beforehand.

1. Includes the following in-house lab tests: Blood Glucose, Influenza, Strep, Urine UTI Screen , Pregnancy, Mono, RSV, H. Pylori, Occult Blood Test. No included: HIV Test
2. Includes the following minor procedures: Blood Draw, Injections
3. Includes one-time medication renewal if applicable

*Note there is a $40 admin fee for each vaccine.
